Senior Diagnostic Center of SouthCentral Michigan (SDC) is a comprehensive health assessment center for older adults and is a partnership of Battle Creek Health Center and Summit Pointe.
SDC is patterned after major medical/university assessment centers and is the only specialized health assessment for older adults in south central Michigan. SDC allows older adults to receive a comprehensive health assessment with out leaving the Battle Creek Community.

Senior Diagnostic Center Health Professionals specialize in Senior Health.
At SDC you’ll participate in a comprehensive physical by a certified Gerontologist, a Geriatric Nurse Practitioner, an Occupational Therapist and Social Worker. The team will work with you in an evaluation of your daily activities, memory assessment, depression evaluation and other personal and physical concerns. A Geriatric Psychiatrist is available when there is need.
The team will develop a comprehensive care plan, which will recommend steps to enhance the quality of your future healthcare and life. The plan will be shared with you and your family at a family conference. That plan will also be provided to your Physician who will help you keep your best foot forward in the days and years ahead. Individuals, physicians or other providers may make referrals.
