Senior Diagnostic Center of SouthCentral Michigan (SDC) collaborates with local providers to enhance the older adults’ quality of life.
The professionals at SDC specialize in gerontology and collaborate with other senior providers for the referrals incorporated in a patients care plan.
SDC provides comprehensive assessments for older adults. SDC is neither a practice providing ongoing healthcare nor a service provider for extended social, psychosocial or in-home care.
Often older adults are referred to SDC to assist in determining their ability to remain independent. The subsequent care plan may suggest that assisted living or select in-home services are necessary to improve their quality of life and keep them safe. The SDC professionals will assist the older adult and their family in arranging those services.
Pamphlets and other helpful information are provided to the patient and their family when service referrals are suggested at the family conference.
The Battle Creek Area has extensive senior services with highly qualified professionals who are available to assist older adults and their families. SDC is proud to be an active member of that network.
