Of Interest to Physicians and other Senior Providers
Senior Diagnostic Center of SouthCentral Michigan (SDC) is a time saving resource designed with a primary care physician’s busy schedule in mind.
Our mission is to provide a resource of collaborative support to busy physicians’ and other professionals who are entrusted with improving the quality of life of seniors. We encourage you to review your current patients and identify those who would benefit from the services provided by SDC. Just call our office and one of our friendly associates will be glad to schedule an appointment for patients you feel are appropriate.
Battle Creek Health System and Summit Pointe have collaborated in the development of this service and are encouraged about the healthy partnership with all local physicians. We are excited about a service that will encourage older adults to pursue this university style assessment in our own community rather than traveling out of town.
SDC has been in operation for approximately 6 years. During this time, they have assessed, developed a tailored care plan and returned over three hundred satisfied seniors and their care plan back to their primary care physicians.
SDC is designed to conserve a busy physician’s time while providing beneficial information that will assist in serving senior patients. Upon referral, patients receive approximately five hours of physical, psychosocial, occupational therapy, pharmacy review and ADL assessment. The health care professionals at the SDC will evaluate the information and communicate the recommendations back to you in a care plan tailored to the individual.
